I understand that to reserve Millard "The Professor" Grubb for my child's party, I simply need to fill out the following information. (After my party information is submitted, I'll receive an electronic confirmation within 2 business days. Should there be a scheduling conflict, The Professor will be in touch with me personally as soon as possible).

I also understand that if The Professor is available and accepts the booking, this form will serve as a booking agreement. I am therefore committed to the date and time I state below. (Unneccesary cancellations will result in a $40 cancellation fee.)

If The Professor is unavailable for the date I request, I understand that I will be given a choice of either changing the date and time of my party, or requesting another magician from the BPP directory.
