I understand that to reserve Paul Amerson for my child's party, I simply need to fill out the following information. (After my party information is submitted, I'll receive an electronic confirmation within 2 business days. Should there be a scheduling conflict, Paul Amerson will be in touch with me personally as soon as possible).

I also understand that if and only if Paul Amerson is available and accepts the booking, this form will serve as a booking agreement. I am therefore committing to the dates and times I state below. (Unnecessary cancellations will result in a $40 cancellation fee.)

If Paul Amerson is unavailable for the dates and times I request, I will be given an opportunity to change the date and time of my party, request another magician from the BPP directory, or cancel altogether (no cancellation fee will be imposed in this case).
